Select the following vertices of the Actuating Coupler with the lasso tool.  *Note that not all the vertices are selected.  The lower left-hand corner and middle vertices are not selected. 
Rotate the vertices to align with the diagram.  It is very sensitive

Create the Shoulder Hub
Create a cylinder with 20 division (defaultdeafault)  rotate and scale it to align with the blueprint above.  This time scale it slightly smaller so that there is a decent gap.

Also position and scale the Shoulder Hub  in the  front view.

This is a two step process.  Select the center faces and extrude.  Change the Offset to 0.035.  This will scale down the faces.
Extrude again. This time change the Thickness to  -0.06 which will push the faces back 0.06 units.

This is also a two step process.  Select the center faces and extrude.  Change the Offset to 0.035.  This will scale down the faces.
Extrude once again. This time change the Thickness to  0.04 which will push the faces back 0.04 units.

As you can see the main leg gets in the way . . . Let’s hide it. 
Select the main section of the leg and press {Ctrl} + {h}
*To unhide, press {Ctrl} + {Shift} + {h}
Another way is to isolate an object.  Select the object(s) and press {Alt} + {i}.  This hides everything else.

We need to see the main leg again . . .
So let’s unhide it.  There are a number of ways:
First try to unhide it, press {Ctrl} + {Shift} + {h} this unhides last hidden object. 
If it doesn’t reappear, you can open the Outliner or Hypergraph, find the leg, select it and then  Display > Show > Show Selection
The last way is to show all Display > Show > Show All

Select the newly extruded Shoulder Hub, Duplicate it  {Ctrl} + {d} and then hide it  {Ctrl} + {h} .
Select the 2nd Shoulder Hub and and scale it as shown above.  We are going to use it as a cookie cutter.
*I scaled it only in the Y direction.

Select the leg and then the stretch out Shoulder Hub. We are going to perform a Boolean operation or in other words, subtract the Shoulder Hub from the leg.  Mesh > Booleans > Difference

Now show the duplicated Shoulder Hub. 
There are a number of ways:
